進行性神経・筋疾患患者の嚥下障害に対する新しい尺度の開発

背景

摂食嚥下障害は多くの進行性の神経・筋疾患において共通して認められる障害であり、誤嚥性肺炎や窒息、栄養障害などを引き起こすため、予後規定因子として重要です。
臨床症状の進行に伴い刻々と変化していく摂食嚥下障害と、長く経口摂取を続けたい患者さんとの想いの間で摂食嚥下の状況も変化していきます。この現状を評価するためには既存の評価尺度では臨床上の細かい変化に対応できず、実情をうまく反映しないことが多くあったため、進行性の神経・筋疾患に共通した特徴を捉えながら汎用性のある尺度で評価することを目的に新しい評価尺度を開発しました。

神経・筋疾患摂食嚥下状況スケール(NdSSS)

障害の進行とともに徐々に食べられるものが制限される過程に加え、濃厚流動液などを補助栄養として経口摂取する特徴や、末期には唾液嚥下も不可能になる特徴などを反映しています。
これまでにDuchenne型筋ジストロフィー症患者１３４名、筋萎縮性側索硬化症患者８４名、多系統委縮症患者８１名において信頼性、並存的妥当性、反応性を評価しました。いずれも高い信頼性、並存的妥当性、反応性を得られ、進行が急性/慢性、疾患が神経/筋疾患に関わらず、進行性の神経・筋疾患患者さんにおける進行性摂食嚥下障害に広く活用できることが示唆されました。
このNdSSSを進行性の神経・筋疾患患者さんに出会う様々な臨床場面で、職種を問わず多くの方々に活用していただき、少しでも長く安心して食べることで患者さんの生活の質が向上することを期待しております。

Level.1	全て代替栄養である上に、唾液の嚥下ができず、唾液の吸引が必要
Level.2	全て代替栄養。唾液嚥下は可能で吸引は不要
Level.3	代替栄養が主体、楽しみレベルの経口摂取をしている
Level.4	栄養剤・食品を主体として経口摂取しており、代替栄養を併用していない
Level.5	３食の嚥下食経口摂取が主体で、不足分を補助的に栄養剤・食品で摂取している
Level.6	３食の嚥下食を経口摂取している。補助的な栄養剤・食品を摂取していない
Level.7	特別に食べにくいものを除いて、３食を経口摂取している
Level.8	食形態の制限はなく、３食を経口摂取している

Level.1	Tube feeding with saliva suctioning in the oral cavity necessary. A patient can neither discharge nor swallow saliva.
Level.2	Tube feeding without suctioning. Although a patient cannot take anything by mouth, can discharge and/or swallow saliva.
Level.3	Tube feeding with occasionally oral intake. A patient sometimes takes orally for the fun, not for nourishment.
Level.4	Totally orally fed and tube free with supplemental nutrients, such as enteral solution. A patient usually takes supplemental nutrients by mouth although don’t take general food.
Level.5	Totally orally fed with easy-to-swallow food and supplemental nutrients, such as enteral solution. A patient sometimes/often takes supplemental nutrients by mouth
Level.6	Totally orally fed with only easy-to-swallow food. A patient eats foods processed in a mixer and drinks thicken water.
Level.7	Totally orally fed with no difficulties. A patient eats without something difficult to eat.
Level.8	Totally orally fed with no restrictions. A patient eats all kinds of food.
